All registration information to be completed is for the Fall 2013 season, to include

Participants must reside in the Pocono Mountain School District; attending any school, or home schooled.

 

Football Eligibility

 

YAPM offers various levels of play based on age/weight groups.

 

Ages are calculated "As of" June 30th, 2013

 

The minimum age is 6 and the maximum age is 13.

 

A 14 year old may participate provided participant has not entered high school.

 

* A 5 year old may be enrolled upon request and at the sole discretion of the YAPM Board of Directors

 

* Team Placement is based on coach's evaluation conducted during the first week of practice.

Every registered child will participate .

YAPM Code of Conduct

YAPM Code of Conduct

 

As a parent/guardian/coach, I am aware that the Youth Association of Pocono Mountain will conduct the sports program with the highest regard for my child’s safety and enjoyment. The association is made up of VOLUNTEERS donating their time and talents. There may be times during the season when you will not agree with the decision/call made by the coach/official/person in charge. We ask that at that time, you conduct yourself in a courteous and respectful manner. 

 

·         Conduct yourself in a manner that is worthy of your position of leadership/responsibility as a parent/guardian/coach.

·         Encourage good sportsmanship by demonstrating positive support for all players, coaches, officials and other parents.

·         Refrain from use of foul or abusive language or gestures towards players, coaches, officials and other parents.

·         Refrain from using any kind of alcohol, drugs and tobacco products on or off the field.

·         NO SMOKING IS PERMITTED ON POCONO MOUNTAIN SCHOOL DISTRICT PROPERTY; THIS INCLUDES YOUR VEHICLE. NO DOGS/PETS ALLOWED ON SCHOOL PROPERTY.

·         Remember that the practices and games are for the children, NOT the parents.

·         Assisting, if asked, any coach/official/parent.

·         Cooperating with the Youth Association of Pocono Mountain and team officials to facilitate the administration of games and practices

·         NO parent will be permitted on the playing field at practice unless his or her child is injured or the parent is called upon. Any questions or grievances will be handled either before or after practice.

·         Please have your child at practice 5 minutes before the scheduled time and arrive 15 minutes before the end of practice in case the coach needs to speak to you.

·         Contributing in any manner to the betterment of the recreation program. (Concession stand, refereeing, committee work, etc.)

·         With respect to concerns or grievances, you should:

o        Speak to your child’s coach

o        Contact appropriate commissioner

·         I also acknowledge that I am responsible for the following:

o        Transporting my child to and from practices and games

o        Cleaning my child’s uniform after each game and prior to returning it to the field at the end of the season, and keeping it free from any damage.

o        Participants are to wear their uniforms to games only, NOT to school.

o        Returning all uniforms and/or equipment loaned to my child/me, or make full restitution of said YAPM property.

·         To help in security during YAPM activities (indoor events or outdoor practices/games). I accept responsibility for controlling my player/siblings from roaming into unauthorized school properties, e.g. bus parking lots, classroom areas etc.

Failure to adhere to the rules and requests outlined within this "Code of Conduct" section will result in the following actions taken against the parents/guardians/coaches.  

·         1st offense: Written Warning

·         2nd offense: Removed from practice/game

·         3rd offense: Banned from practices and/or games

·         4th offense: Ban child from all participation in that sport

Consent Release

Photo Consent

Photos of participants without references to their name may be published without permission, but it is against YAPM policy to publish any individual participant’s photo with a reference to their name without specific permission.

 

Email Consent

Members primary email address, as entered above, will be automatically entered into the YAPM email notification database. The YAPM relies on this means of communications to keep members informed on changes in schedules and event reminders. I understand that this system is provided as a courtesy to parents and guardians of YAPM participants. The YAPM does not guarantee or confirm delivery of messages sent to registered recipients of this service.

 

Your information will be used solely for the notification of events relating to the YAPM.

Responsibility & Release Agreements

PLEASE READ CAREFULLY

 

"No Refund" policy of the YAPM

No refunds will be made for the mandatory fundraiser costs, however, refund of the registration fee is only feasible prior to uniform handouts/equipment handouts following written requests to the YAPM Board of Directors. If withdrawal from the program is necessary, you must return your equipment/uniform prior to submission of your refund consideration request. 

 

Equipment and Return Policy

I the Parent/Guardian understand that I am liable for any and all equipment or moneys from fundraising projects assigned to my child/children in order for them to participate in this community program. If any of these items are not returned to the YAPM by December 1st, 2013, then I am obligated to pay the replacement cost. I realize that I will be summoned to the District Magistrate’s Office for collection of such moneys in addition to all fees associated with the recovery of YAPM property which includes, but is not limited to, court costs, filing fees and constable servicing charges.

 

Risk of Injury Indemnification

I recognize the possibility of physical injury associated with football/cheerleading and in consideration of the YAPM accepting the registrant(s) for it’s football/cheerleading programs and activities, I hereby release, discharge and/or otherwise indemnify the YAPM, its affiliated organizations and sponsors, their employees and associated personnel, including the owners of the fields and facilities utilized for the programs, against any claims by or on behalf of the registrant(s) as a result.

 

Proof of Eligibility - First Year Participants

New participants must provide, to the Youth Association of Pocono Mountain, a copy of your child’s original birth certificate and a recent photo of your child. You will also be required to present the original birth certificate to a league official, at a date and time to be announced, for copy comparison and verification to insure compliance with league age restrictions.

 

I, the Parent/Guardian of the registrant(s), a minor, agree that the registrant(s) and I will abide by the rules of the YAPM, its affiliated organizations and sponsors.

 I will pay registration fee by
